What is it?

eSalut is a project of the Catalan Institute of Health in which, through a mobile application, personalized information is offered to patients to help them monitor their diseases from home.
The mobile application facilitates communication between the patient and the professional, while involving the user in their care process and improving their quality of life.

Who can access?

Only those who have been registered by the healthcare team within the application can use it. eSalut is available on both the Apple Store and the Google Play Store. The first to test this tool have been the users of the Germans Trias Hospital and some Primary Care Centers (CAP) of North Metropolitan Primary Care in Badalona.

 

What can the patient find in the application?

The patient can find information, advice, guidelines and personalized exercises, designed for each case. The contents have been prescribed, prepared and validated by your healthcare team. The tool helps to improve patient monitoring and marks the most appropriate control times in a personalized way, depending on the clinical situation of the patient at all times.
Currently, eSalut already has contents for 60 different pathologies or processes, such as persistent covid, cardiac surgery, colorectal cancer, pain, stroke, diabetic foot or home hospitalization, among others.

 

What does the application allow to do?

►With the app, professionals propose to patients tasks to be carried out every day, such as indicating which symptoms they have, what evolution they are noticing or what exercises -with videos of respiratory rehabilitation, or mobility- they must continue to improve. They also propose challenges, materials and resources, adapted to their illness, so that they have more knowledge.
►Patients can find forms to answer questions about their quality of life or their habits and, when they visit them, they already have their team of professionals who already have this updated information.
►The virtual space allows to bring together all the pathologies that a patient can have in a single personalized tool, so that all the tips reach the patient evenly by a single channel.
►The application collects the information that the patient is registering and can share it with professionals from the Hospital and primary care. The user can also communicate in an agile way with their care team.

All this allows the team of professionals to monitor the health processes of their patients more closely.
